How It Works
Schedule your in-person or telehealth visit through the online calendar, or by leaving a message at (503) 897-7102. If you can do an in person visit, that's what I recommend - this gives us more options for examination and making sure we have a recent blood pressure.
Once we confirm your appointment, you will receive an email invitation to the Menopause Northwest patient portal.
Fill out registration and health history forms online in the patient portal prior to your visit.
Payment and Insurance
I am in network with the following insurance companies and can bill them directly for your visit*.
Blue Cross Blue Shield of Oregon
I cannot see patients with Medicare coverage, even for a self-pay visit. This is a Medicare rule.
If you have insurance through a plan that is not listed above, you are still welcome to schedule an appointment with me. You can submit your bill from us to your plan for any out-of-network coverage or FSA reimbursement that you qualify for.
If you wish to self-pay for your care instead of using insurance, our prompt self-pay discount is available to you.
*Please call your insurance company and check the coverage for your specific plan before your first visit with me. Even if I am in your network you may have a deductible to pay before your insurance will cover your care.
