Payment and Insurance*
My network participation in most insurance plans is ending. I can provide a superbill for you to submit to your health insurance plan for out of network benefits.
I currently cannot see patients with Medicare coverage, even for a self-pay visit.
*If you believe you have coverage for your visit with me, Please call your insurance company and check the coverage for your specific plan before you see me. Even if I am in your network, you may have a deductible to pay before your insurance will contribute financially your care.
